Healthcare Patient Privacy Statement

THIS NOTICE DESCRIBES HOW CBSF PROTECTS OUR PATIENT MEDICAL INFORMATION. PLEASE REVIEW IT CAREFULLY.



Every CBSF Mission is carefully planned and recorded. A record of the patients that we administer to is also recorded and is not shared with anyone unless specifically requested by the patient. In the event that we make a referral to a near-by hospital or clinic, we obtain to the best of our ability the convent to share information with the named hospital of clinic. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment, a plan for future care or treatment, and billing information. This notice applies to all of the records of your care generated by us, whether made by your physician or our employees. This Notice of Privacy Practices is being provided to you as a requirement of the Health Insurance Portability and Accountability Act (HIPAA). While we are not asked to follow the US standard of (HIPPAA), we put into practice its policies as they apply. We work solely as a private mission to serve the underserved population of the Northwest of Haiti in collaboration with the Ministry of Health in Haiti. This Notice describes how we may use and disclose your protected health information to carry out treatment, payment, or health care operations and for other purposes that are permitted or required by law. It also describes your rights to access and control your protected health information in some cases. Your "protected health information" means any of your written, electronic, and orally transmitted health information, including demographic data that can be used to identify you. This is health information that is created or received by a health care provider and relates to your past, present, or future physical or mental health condition.



Although we are not required by law to maintain the privacy of your protected health information and provide you a description of our privacy practices. We abide by the terms of this Notice.



Caring Brothers and Sisters Foundation, Inc. Is privately funded by donations and volunteers and works in compliance of the Haitian Ministry of Health and puts into practice Modern compliance and ordinances of best practices that are recognized internationally and in the US.
